Case Management

"The people at BCAP always treat me with respect. I never feel like I'm just another case."

Who We Serve
Boulder County AIDS Project (BCAP) serves men, women and children who are HIV positive living in Boulder, Broomfield, Clear Creek and Gilpin Counties. All care services are free, confidential and offered within a non-judgmental environment. Care services include case management, access to medical care, financial assistance, food bank, volunteer support, support groups, and pro bono professional support services.

BCAP is a welcoming place. We celebrate and value each person entering our doors for services regardless of age, race, culture, gender, gender identity, gender expression, sexual orientation, lifestyle, background, religion, ability, or income.

Newly Diagnosed? New to the Area?
Learning about an HIV diagnosis or relocating to a new community with HIV can be an overwhelming experience. If you seek case management services, your first step is to contact us at 303-444-6121. A staff person will complete a Request for Care Services with you right away. They will ask you some basic questions and get information on how to best reach you. They will also ask you how you want us to identify ourselves when we call you back to schedule an intake appointment (e.g., Can we say BCAP? Should we just mention our first name?). By guiding us this way your confidentiality can be guaranteed.

A case manager will call you back within two business days of receiving your Request for Care Services to set up an appointment with you. You will also be asked to gather paperwork that will help us verify your eligibility for services.

When coming for your intake appointment, please bring:

  • Proof of HIV diagnosis from your clinic or physician (copy of labs with T-cell count and viral load; written statement from clinic or physician)
  • Proof of address (a utility or phone bill, lease)
  • Photo ID (Colorado driver’s license or ID)
  • Proof of health insurance (Medicaid, Medicare, insurance card or CICP card)
  • Proof of your income (e.g., check stubs, bank statements, disability award letter)

The case manager will help guide you through the intake process. If you are homeless or lack income, just let your case manager know. You can help us best serve you by bringing a list of your questions. Your case manager will review your questions and needs, and together you will make a plan to address your needs.
